Are there limits to the number of consecutive swaps/passes?

I was playing recently with a friend and the game suddenly shut down and declared me the winner, even though we both had plenty of letters to play.  we think it was because we might have done too many swaps and/or passes in a row, or perhaps too many during the entire game? We can't recall the exact number but that seemed to be the reason.  Are there limits to the number of swaps/passes allowed and if so what are they?

Three consecutive passes results in row results in the game ending. I am not sure if three swaps in a row result in the game ending or not.

I've played Pass and Play a few times and swapped a bunch in row just to see what would happen. Nothing happened. So: that's unlimited I believe (but boring)
Nope--swapping 3x ends in too :( Just learned the hard way--uggh
