Recent game ended with 12 letters left in draw pile, why?

I had 7 letters left , my opponent had 7 letters left, there were 12 letters left in draw pile.  She had passed twice, but i still had valid words to play.  Why did the game end and give her the win?

Did you pass once when she passed twice? If there are three passes in a row, the game ends.

Thanks for replying Debby. She passed, I swapped three letters, she passed - game ended!  After my swap, I then had valid plays.  It was a terrible board!!
